What the quote form collects

Information we ask for

When you submit a quote request we collect your name, work email, organization, optional phone number, the product category and the message you write. We also keep a reference for the request.

If you attach a supply list, we store the spreadsheet privately and link it to your request. File attachment requires a signed-in account; your authentication provider processes account and sign-in information. Download links require the account that uploaded the file. Do not upload sensitive patient information.

How it is used

Submitted details are stored so your request can be reviewed. We use Resend to send inquiry details to our team by email. Uploaded files remain in private storage and are not included in notification emails. Submitting a request does not create a purchase or any commitment on either side.

No patient information

This site is for supply inquiries only. Please never enter or attach patient names, results, identifiers or any other protected health information.

Sharing

If you opt into optional quote analytics and analytics is enabled, we send Google Analytics a successful-request event with a fixed page address and title. We do not include your contact details, request reference, message, file information or query parameters in that event. Declining does not prevent an inquiry.

We do not sell your details. This notice is a plain summary and will be expanded as the service develops.
